50 mile radius rule for travelers
Local Candidates accepted - local rate is $68

FLU VACCINE IS REQUIRED at UMMC - only medical exemption is allowed and requires documented evidence from a health care provider of a past severe reaction to the vaccine

FINGERPRINT REQUIREMENT - Remote process must allow 10 days, In Person can be done at Jackson or Grenada campus the week prior to start

MUST HAVE EXPERIENCE:
Must be RN Case manager with at least 2 years Acute Care Management experience in discharge planning experience.
Experience must include at least 6 mos at a level 1 or large academic Medical Center hospital working with a variety of inpatient units for medical discharge
EPIC CHARTING experience is REQUIRED
5x8hr shifts - non-negotiable
General Certification Requirements (BLS, etc.) BLS, RN Case Manager
Other Certifications/Specialty certifications requirements? CCM preferred but not required
Prefer RN, BSN, *Would consider ADN .
Department: CM - General Case Manager/Discharge planning

Case load/# patients per day/consults per day 15-20
EMR - Will use naviHealth to send referrals and plan to switch to EPIC Care Management over the next year)
Do you require InterQual criteria or Milliman experience - No, but need to have general understanding of discharge readiness
Is this position for UR, discharge planning, or both? Case Management - Discharge planning
What areas do they cover (Inpatient, Outpatient, ER, Home Care, etc.) Will cover all Inpatient units throughout the hospital

Currently 15 CM's on the CM team, goal is to have 21 (18 general CM's and 3 Complex CM's).
RN's work with Social Work team which consists of 15 full-time and 17 part-time Social workers.

Common types of patients & job duties Medical discharge (could be some social discharges)
UMMC has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met. Criteria has a limitation on financial assistance for only those uninsured or underinsured.
UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.
CM Manager/Supervisor Department does not have a CM Manager at this time. Will directly report to the Social Work Manager. Chief of Care Coordination is also providing department oversight as needed.
Shifts & Scheduling ? M-F 8-430p
Weekend Requirements None
Holiday Requirements Rotated as needed
Scrub Color/Dress Code Any color scrubs or business professional attire
RT for DC Planning? Will consult with RT as need
Pharmacy for DC Planning? Will consult with Pharmacy as needed
Department Specific Orientation 2 weeks (will include general facility orientation) Will work alongside CM team so will never be working alone.

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
