Description

After Hours Registered Nurse - Phone Triage
Primary Purpose: Provide professional assessments, prioritizing treatments according to the urgency of need, utilizing excellent critical thinking skills, while being able to remain independent in the application of nursing knowledge and skill. Manage urgent issues in collaboration with Primary Care Provider with paramedics, pharmacy staff, administration staff, and hospital transitions to best meet the patient’s needs and goals.
Duties and Responsibilities:

Uphold and support the mission, core values, objectives, and policies of SCPP PACE.
Respond promptly to each incoming call.
Perform patient assessments to determine the appropriate level of care required to safely meet medical needs according to the participants plan of care.Emergent Urgent Non-urgent Home care status
When appropriate, provide home care instructions using the approved, written guidelines as well as approved reference material provided.
Communicates and collaborates urgent issues with paramedics, pharmacy, administration, home care coordinator and primary care provider on call, clearly and effectively.
Initiates emergency treatment if necessary.
Initiates hospital transition including faxing of current MAR, DPOA and Code status paperwork to medical facility.
Thorough and completed documentation utilizing the appropriate computer software in compliance with the approved policies and procedures.
Actively participate in new employee orientation, ongoing in-service programs, staff meetings, continuous quality improvement, and periodic performance /protocol evaluations and development.

Qualifications:

Graduate of an accredited School of Nursing.
Current Nurse licensure in the State of Michigan (LPN, RN)
Minimum of one year of experience working with the frail, elderly or long-term care population.
Minimum of one year of home care experience.
Minimum 2 years working in nursing.
Participation in continuing education programs.
Familiarity/experience with participant interactions on telephone.
Strong ability to establish and maintain effective working relationships with participants, coworkers, staff, and medical service providers.
Effective communication and people skills.

Specific Requirements:

Maintain current nursing licensure by completing applications for renewal in a timely manner and by complying with all requirements for continuing education.
Has time-sensitive clinical decision-making skills, critical thinking competencies and an ability to accurately identify participant problems.
Provides exceptional customer service in support of staff, external partners, and participants.
Possesses the ability to make independent nursing decisions.
Strong ability to communicate effectively both verbally and in writing; thoughts are logical and clearly expressed.
Must be able to work well under stress and relate to the public, nurses, and physicians in a tactful manner.
Must have adequate typing skills and basic computer knowledge.
Maintain a work environment that allows them to be fully present for their shift, with adequate cellular and internet service.
The After-Hours Registered Nurse will have access to a computer terminal, and telephonic equipment to be used in remote/home office.

Work Environment:

This position can be done remotely.
Communicates with staff, participants, caregivers of Senior Care Partners PACE and other organizations as necessary.
Is subject to hostile and emotionally upset participants, family members, personnel, and visitors.

Physical Demands:

Constantly operate computer equipment that requires manual dexterity and programs that require color distinction.
Requires stationary position for much of the time.
Communicates via telephone and exchange accurate information with callers 100% of the time.
Must be able to cope with the mental and emotional stress of the position.
Must function independently, and have flexibility, personal integrity, and the ability to work effectively with participants, personnel, and support agencies.

Minimum Working Requirements:

Must have reliable transportation, current valid Michigan driver’s license and car insurance.
Must be 18 years of age or older.
Must be fully vaccinated, or willing to be fully vaccinated against the following communicable diseases: Influenza, MMR, Varicella, Tdap/Td, Covid-19 and Hepatitis B.

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
