Data Collection : Unit Information:   ( insert name ) Location: HCA Houston Clear Lake Address: 500 Medical Center Blvd. Floor: Respiratory Therapy Building:   General Information Tell us about the unit(s): ·        Number of Beds - 540 ·        Number of Staff – 60 ·        Type of staff: Respiratory Therapy o    Day Shift – ( example 1 charge, 3 RN, 2 CNA, and 1-3 sitters) 10, 1 free charge RT o    Night Shift - 10, 1 free charge RT ·        Patient Ratios – 6 vents/RT ·        Type of equipment – Hamilton C1-G5 vents, Teleflex HFNC, BiPAP V60s ·        EMR - Typical hiring profile Skill Set Most recruited for:     Must have :   candidates without these skills will not be considered for the role. ·          NRP ·        1 year Level III/IV NICU experience Preferred or nice to have :   candidates with these skills will be considered first. ·        Adult and NICU trained ·          List typical procedures performed on unit(s):   ·        Assist with high-risk deliveries ·        Neonatal resuscitation ·        Operate oscillator/jet specialty vents     Best personality Fit: ·        Team-oriented, self-motivated REQUIRED CERTIFICATIONS: ·        NRP Must have Floating Area(s): ·          Dress Code : ·        Scrub Color/Attire – Pewter gray   Unit Guidelines/Policies : Please feel free to attach unit policies and guidelines. ·            Weekends and On call(if any) ·        Every other
