Position / Specialty: Case Manager / Care Navigation RN Start date: Ratios: Days1:20-24, Weekends 1:80 with focus on discharging patients Years of experience REQ: 1 year First-timers accepted: (Y/N) Yes - Need experienced discharge planner Weekend REQ: Yes – every other Certs REQ: BCLS is preferred. Is on-call REQ? (Y/N) What is the req? N Will clinician float within scope to meet facility needs? Yes, required Floating between Toledo, Flower, Bay Park, & Wildwood facilities as needed. Open to accommodating block schedule? (Y/N) N Locals accepted: (Y/N) What is the local radius rule? No locals travelers 50 miles or less Pending License accepted: (Y/N) Yes RTO Restrictions: Greater than 3 days will require Management approval Guaranteed Hours: 40 Special Requirements: - Patient Age group: Adults and Geriatrics - Patient Diagnoses • Wide range of diagnosis related to trauma, cardiovascular, renal, stroke, post surgical, respiratory, oncology, CHF and vascular diagnosis. - Special Procedures/Unit Details • Teaching hospital with Residency program, • Level 1 Trauma, Level 1 Neuro with various units in Tertiary Care Hospital including: Trauma and stepdown unit, Neuro ICU and acute neuro medical and surgical intermediate floors, Cardiac and Medical Acute/ intermediate and ICU, Post Open Heart CV Acute and ICU, Oncology, Progressive with ventilator capability, Orthopedics acute, Adult and Medical ICU, Neuro Medical and Surgical units, Stroke Acute and Intermediate and ICU Medical Surgical Acute- post op, vascular and hemodialysis population - Special Equipment • Laptops - Skills Required Epic and CarePort experience Acute Hospital Work Setting Required Required Case Management/Utilization Review Skills: Care coordination, Discharge Planning, Needs Assessment/ Order DME, Prior Authorizations • Care Navigation/ Discharge Planning • Charge Experience – preferred • Teaching Hospital Experience – preferred • Community Hospital Experience – preferred • LTAC Experience – preferred • Trauma Level I Experience – preferred Hospital Highlights Type of Facility: Level I trauma center and the largest acute care hospital in the Toledo metropolitan area Magnet / Teaching / Level Trauma (if applicable): Level 1 trauma center Total Staffed Beds: 124 Dress Code: Business casual for Care Navigation. We are able to wear a white lab cover jacket. No open toes. Charting: EPIC and CarePort Parking Cost: Free

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
