Licensed Social Worker
TACT Staff is seeking a dedicated and compassionate Licensed Social Worker to join our team! As an LSW, you will be responsible for providing direct social work services to individuals, families, and groups in need. The ideal candidate for this role should possess excellent communication and interpersonal skills, be able to work with diverse populations, and have a strong understanding of evidence-based practice.
Responsibilities:
- Provide medical case management to patients living with HIV/AIDS
- Complete intakes, psychosocial assessments, and continued needs assessments
- Develop care plans for appropriate client treatment.
- Provide counseling and mental health support as necessary.
- Conduct mental health, oral health, domestic violence, sexual abuse, and substance abuse screens. Make appropriate referrals when required.
- Collaborate with interdisciplinary team in the update and reassessment of psychosocial and medical needs, as well as discussion of treatment plan.
- Assist patients in securing proper medical and mental healthcare.
- Reinforce self-care and treatment adherence, including risk reduction counseling and education.
- Coordinate care with other community agencies and systems.
- Serve as a liaison between the patient, healthcare team, and other community agencies.
- Assist patients in navigating the health, insurance, and public assistance systems.
- Assist in removing barriers that impede access to medical care.
- Provide community referrals as needed.
- Gather information necessary for continued grant funding.
- Active LCSW or LSW licensure for New Jersey State.
- 1+ year of professional experience post licensure.

Nicole was very helping and always proficient with reaching out and even going above and beyond when the facility itself was slacking. When the facility canceled all contracts the day before start date she found me another position within the same week. I would recommend her 10x over.
