License- Active and in hand - AZ or Compact- Ensure it's accurate # of suites/labs 1 suite and 1 room (used for less complex cases) #cases per day on average 25-32 per day Required Certifications (i.e., BLS, ACLS?) Does the charge nurse take an assignment? "Manager: for additional concerns and resource, and a lead tech who handles more in rooms needs " Shifts & Scheduling (Do you allow self-scheduling? Block scheduling?)
