Overview:We are looking for a Nurse Practitioner Preceptor - AIM (Advanced Illness Management) Program. This position will directly report to the VP of Palliative Clinical and is responsible for providing high quality interactions and training with newly hired or tenured NPs during palliative care/AIM visits across the Palliative division. The coverage area would be Pennsylvania - candidate must live and be licensed in PA. Remote/Travel.
- Facilitates the learning experience to allow preceptees to perform competently in their required responsibilities. This is achieved through mentoring, coaching, and role modeling correct behaviors.
- Performs patient home visits as needed
- Orients new or trains tenured NPs (preceptees) which includes palliative clinical policies and procedures, documentation, value-based required patient outcomes, technology, and compliance and regulatory requirements.
- Performs ongoing metric monitoring to provide ongoing coaching and feedback directly or indirectly with preceptees.
- Advanced Practice Practitioner required
- Unencumbered NP and RN license in Pennsylvania - must live and be licensed in PA
- Active DEA license
- Current CPR certification
- Minimum 5 years of experience in a clinical setting
- Experienced in part B documentation requirements
