The Home Health Aide provides patient care services under the supervision of a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Pathologist in accordance with an established plan of care and per agency policies/procedures. This position requires population specific competencies. Adheres to National Patient Safety Goals as appropriate based on the level of patient contact this position requires. * Certification by Medicare Standards prior to onset of job responsibilities and must be on the Home Health Aide Registry of the State Department of Health
* Certification as a Home Health Aide in the State of Oklahoma, or must pass a written and skills exam to become certified
* CPR certification
* 1 year of nursing experience or home health aide experience
* Previous home health experience preferred
* This job requires the incumbents to operate an INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Oklahoma State Drivers License as well as a driving record which is acceptable to our insurance carrier.
The Home Health Aide responsibilities include, but are not limited to, the following: * Calls patients and schedules visits in coordination with established plan of care, supervisor of care and the scheduling department * Performs patient care as directed by the care plan and patient tolerance level for that day * Provides a patient report to the supervising clinician assigned to the patient after each visit * Maintains a clean and safe environment by changing bed linens, cleaning the immediate area around the patient, patient's bathroom in relation to the patient?s personal care, light housekeeping, etc. * Prepares progress notes for each visit and completes necessary paperwork assignments to document patient care activities as a part of the medical record The Home Health Aide reports to the Team Manager or Clinical Manager. This position may have additional or varied physical demand and/or respiratory fit test requirements. Potential for exposure to infections and communicable diseases, blood and body fluids, electrical equipment, chemicals. Must follow standard precautions.
