Why Home Health"When the time comes to discharge a patient, it can be difficult because you created a special bond with them and their family and caregivers, as they allowed us the honor of coming into their homes to walk side by side with them. This is the moment I love to tell them how rewarding it has been, as they are now able to "graduate" from home care and begin living their best lives!"~St. Luke's Home Health RN
