Description
At St. Luke’s, our dedicated team of Hospice RN Liaisons strive to build a positive, supportive, and inclusive culture that delivers exceptional patient experiences.
Nurse Liaisons are a critical role for the success of all post-acute care. A successful liaison will have great people skills, be able to build trusting relationships with hospital staff and providers and and have a true passion and desire to care for end-of-life patients to ensure their needs are met. This position requires travel within Twin Falls and it's surrounding areas.
What you can expect from this role:
The Liaison will coordinate and facilitate referrals for end-of-life patients. Additional responsibilities include scheduling initial assessments and admissions, establish patients’ eligibility for care, review referrals for appropriate placement, coordinate discharge and post discharge requirements, meet with patients/families to describe our services and educate them on the benefits of hospice care. You will also work closely with providers and staff for other possible referrals.
- Full Time Days (Mon-Fri) with rotating weekends
- Develop and sustain strong working relationships with staff who refer to St. Luke's to increase referrals and admissions
- Participate in marketing and education programs to increase awareness across the system

Qualifications
- Education: ASN Degree
- Experience: 1 year of RN experience, preferably in an Liaison role; Home Health, Acute Care and/or Case Management experience helpful
- Licenses/Certifications: Current RN License in the State of Practice; current Basic Life Support (BLS) Certification
- Valid Driver's License and proof of Auto Insurance
