Job Description

• Role: Physical Therapist for Home Health
• Responsibilities:

  • Conduct home visits for therapy.
  • Participate in periodic office meetings.
  • Must have reliable transportation, a current driver's license, and agency-required liability insurance.
• Average Patients per Day: 5-6
• Points System:

  • 30 points per week, with each type of visit and activity (documentation, travel, etc.) holding a different point value.
  • Assigned between 5-10 patients per day, equating to roughly 6-8 points.

Requirements

• Experience: Minimum of 1 full year of Adult Home Health experience.
• First-timers: Not accepted; must have recent Home Health experience.
• Certifications:

  • CPR
  • BLS
• On-call Requirement: Varies per job/location; to be discussed during the interview.
• Weekend Requirement: Varies per location and profession; to be discussed during the interview.
• Territory: Must be willing to travel up to 75 miles from the home clinic.
• Local Candidates: Accepted at the same rate.

Additional Information

• Mileage Reimbursement: IRS Standard rate; compensated from patient home to patient home (commute to/from the first and last patient is not billable).
• EMR System: HCHB
• Types of Cases: Start of Care and Follow Ups
• Weekly Productivity Expectations: 28-30 units

Compliance and Documentation

• Candidates must complete various compliance and health documents prior to starting, including:

  • Background checks
  • Health documents (e.g., TB test, vaccinations)
  • Certifications and training related to home health and hospice care.
