Job Opportunity: RN Telemetry I

Overview

Specialty: RN Telemetry I
Location: Chicago, Illinois
Facility: Resurrection Medical Center, part of Prime Healthcare Foundation, Inc.
Employment Type: LTA (Long-Term Assignment)
Contract Length: 25 weeks
Shifts: 5 nightshift Per Diem Tele RN positions available.

Required Credentials and Certifications

Driver's License/State ID: Required for onboarding.
Prime OIG: Required for onboarding.
Prime Drug Screening: Minimum 10-panel test required within 60 days of start.
Prime Color-blind Test: Must be completed annually.
Prime Agency Attestation: Required for onboarding.
Prime SAM/EPLS: Required for onboarding.
BLS Certification: Must be from American Heart Association or Red Cross.
ACLS Certification: Must be from American Heart Association or Red Cross.
NIHSS Certification: Required for onboarding.
Prime Basic Pharmacology Exam: Must score 80% or higher.
Prime Specialty Exam: Must score 80% or higher.
Prime Education Verification: Required for onboarding.
COVID-19 Vaccine: 2 vaccines and booster required or declination completed.
MMR Immunity: Proof of 2 vaccines or positive titer required.
Fit Test: Annual respirator fit test required.
Hepatitis B Immunity: Required for onboarding.
Influenza Vaccine: Required from October 1 to May 31.
TB Test: Required for onboarding.
Varicella Immunity: Vaccination or titer showing immunity required.
Tdap Immunity: Required for onboarding.
Annual TB Questionnaire: Required for onboarding.
Social Security Card: Required for onboarding.
Physical/Health Statement: Must be completed annually.
Prime Core Competencies Exam: Must score 80% or higher.
Prime Skills Checklist: Required for submission.
Prime Criminal Background Check: Required for onboarding.
Prime Badge Photo: Clear headshot required for onboarding.
Prime Form I-9: Employment eligibility verification required for onboarding.
Prime Job Description: Must be signed by employee.
Prime COA: Required for onboarding.
DCI/BVP Certification: Required for onboarding.
Essentials Onboarding Packet: Required for onboarding.
