Overview

Facility: Resurrection Medical Center, part of Prime Healthcare Foundation, Inc.
Employment Type: LTA (Long-Term Assignment)
Contract Length: 12 months

Required Credentials and Certifications

Driver's License/State ID: Required for onboarding.
Prime OIG: Required for onboarding.
Drug Screening: Minimum 10-panel test required within 60 days of start.
Color-blind Test: Must be completed annually.
Agency Attestation: Required for onboarding.
SAM/EPLS Check: Required for onboarding.
Resume: Required for submission.
References: Two references required for submission.
Name Verification Affidavit: Optional for onboarding if there are name discrepancies.
BLS Certification: Required from American Heart Association or Red Cross.
PALS Certification: Required from American Heart Association or Red Cross.
ACLS Certification: Required from American Heart Association or Red Cross.
NIHSS Certification: Required for onboarding.
Basic Pharmacology Exam: Must score 80% or higher.
Basic Rhythm Interpretation Screening: Must score 80% or higher.
Specialty Exam: Must score 80% or higher.
Education Verification: Required for onboarding.
COVID-19 Vaccine: 2 vaccines and booster required or declination completed.
MMR Immunity: Proof of 2 vaccines or positive titer required.
Fit Test: Annual respirator fit test required.
Hepatitis B Immunity: Required for onboarding.
Influenza Vaccine: Required from October 1 to May 31 or declination.
TB Test: Required for onboarding.
Varicella Immunity: Vaccination or titer required.
Tdap Immunity: Required for onboarding.
Annual TB Questionnaire: Required for onboarding.
Social Security Card: Required for onboarding.
Physical/Health Statement: Must be completed annually.
Core Competencies Exam: Must score 80% or higher.
