Job Description

This position requires the ability to perform call duties and handle heart and lung transplants, as well as all cardiac procedures. Scrubbing experience is a plus.

Required Credentials

1. BLS (Basic Life Support)

   • Required for onboarding
   • American Heart Association certification only

2. ACLS (Advanced Cardiovascular Life Support)

   • Required for onboarding
   • American Heart Association certification only

3. License/Certificate Verification

   • Primary source verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are not accepted.

4. MMR (Measles, Mumps, Rubella)

   • Positive titers required from within the last 15 years.

5. Fit Test

   • Must be completed within 1 year prior to start.

6. Hepatitis B

   • Proof of vaccination series or signed declination required.

7. Varicella

   • Positive titers required from within the last 15 years.

8. Tdap (Tetanus, Diphtheria, Pertussis)

   • Proof of vaccination within the past 10 years required.

9. 2-Step PPD or IGRA (Quantiferon/Tspot)

   • Negative reads required within the last 6 months.

10. Physical/Health Statement

    • Pre-employment exam statement required, indicating ability to work without restrictions.

11. Influenza Vaccine

    • Proof of vaccination required from October to March, or approved exemption.

12. UHS I-9 Attestation

    • Completed I-9 or attestation required.

13. UHS Education Verification

    • Highest level of education must be verified.

14. UHS Employment Verification

    • Verification of employment within the last 5 years required.

15. UHS Skills Checklist

    • Skill-specific checklist to be completed within 1 year prior to start.

16. UHS Competency Exam

    • Must score at least 80%, completed within 1 year prior to start.

17. UHS Core Competencies

    • 80% passing score required, completed within 1 year prior to start.

18. UHS Criminal Background Check Results

    • Must be completed within 6 months of start.

19. UHS OIG Search

    • To be completed within 6 months prior to start.

20. UHS Drug Screen

    • Must be completed within 60 days of start.

21. UHS Module Completion Certificate

    • Safety and compliance training modules must be completed prior to clearance.
