Job Opportunity: Pharmacist

Overview

• Specialty: Pharmacist

• Employment Type: Long-Term Assignment (LTA)
• Contract Length: 12 months

Work Schedule

• 40 hours per week
• Evening shifts: 5x 8-hour shifts
• Overnight shifts: 10-hour shifts

Experience Requirements

• 1-2 years of hospital experience preferred.

Credentialing Requirements

Required for Onboarding

1. License/Certificate Verification: Primary Source Verification of professional license and/or certification.
2. MGB NWH Self-Study Orientation & Attestation: Completion required.
3. COVID-19 Vaccine: Completed primary series or up-to-date vaccination required.
4. MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity.
5. Influenza Vaccine: Required during flu season (Sept 15 - April 30).
6. TB Test: Documentation of TB skin test screening or negative QuantiFERON TB Test within 3 months of start date.
7. Health Statement: Signed by MD/DO/NP/PA within 365 days of start date.
8. MGB Criminal Background Check: Required for submission.
9. MGB Massachusetts CORI: Criminal history check required.
10. MGB Resume: Include past 7 years of work experience.
11. Respirator Medical Clearance (RMC) Attestation: Required.
12. MGB Education Verification: Verification of highest level of education achieved.
13. MGB SSN Trace: Required for background check.
14. MGB National Sex Offender Check: Required.
15. MGB Exclusion Checks: Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

Optional but Preferred

• Hepatitis B Vaccination: Record of 3 vaccines or proof of immunity.
• Tdap Vaccination: Updated within 10 years.

Additional Information

• References: Minimum of 2 recent references, including at least 1 supervisor.
• Skills Checklist: Required for submission.
• Confirmation of Assignment/POI Form: Required for all initial assignments and extensions.
