Overview

Specialty: RN Behavioral Health
Location: Woonsocket, Rhode Island
Facility: Landmark Medical Center, part of Prime Healthcare Foundation, Inc.
Employment Type: LTA (Long-Term Assignment)
Contract Length: 12 months

Job Description

Schedule: 40 hours per week, 5 shifts of 8 hours each (6:45 AM - 3:15 PM), every other weekend.
Experience Required: Minimum of 2 years of Behavioral Health Unit (BHU) RN experience with adult population.
Licensure: Current Rhode Island or compact RN License required at the time of submission.
Certifications: BLS (Basic Life Support) certification required.

Required Credentials

Driver's License/State ID: Required for onboarding.
Prime OIG: Required for onboarding.
Prime Drug Screening: Minimum 10-panel test required for onboarding.
Prime Color-blind Test: Must be completed annually for onboarding.
Prime Agency Attestation: Required for onboarding.
Prime SAM/EPLS: Required for onboarding.
COVID-19 Vaccine: Proof of 2 vaccines and booster required for onboarding.
MMR (Measles, Mumps, Rubella): Proof of vaccination or positive titer required for onboarding.
Fit Test: Annual respirator fit test required for onboarding.
Hepatitis B: Required for onboarding.
Influenza Vaccine: Required from October 1 to May 31 for onboarding.
TB Test: Required for onboarding.
Varicella: Proof of vaccination or titer required for onboarding.
Tdap: Required for onboarding.
Physical/Health Statement: Required for onboarding.
Prime Core Competencies Exam: Must score 80% or higher for onboarding.
Prime Skills Checklist: Required for submission.
Prime Criminal Background Check: Required for onboarding.
Prime Form I-9: Required for onboarding.
Prime Specialty Exam: Must score 80% or higher for onboarding.
Prime Facility Documents: Required for onboarding.

Additional Information

A notarized name affidavit is required for any discrepancies between credentials (optional).
JPEG photo required for badge (optional).
