• Employment Type: LTA (Long-Term Assignment)
• Contract Length: 157 days
• Shift: Rotating shift, full-time (40 hours a week), working every other weekend.

Required Credentials and Certifications

1. BLS Certification

   • Required for all direct patient care personnel.
   • Attach scanned copy of certification (with expiration date).

2. License/Certificate Verification

   • Must be dated within 30 days of start.
   • Attach scanned online verification page.

3. Fit Test

   • Required for onboarding.

4. Trinity 9 Panel Drug Screen

   • Must be completed within 30 days of start date (preferred within 14 days).
   • Must include specific substances (e.g., Cocaine, Amphetamines, Opiates, etc.).

5. Trinity Code of Conduct Form

   • Must have page 27 signed form on file.

6. Trinity I9

   • Standard I9 completed by the agency.

7. Trinity Confidentiality Agreement

   • Must have page 5 signed form on file.

8. Trinity Integrity and Audit Services Form

   • Required for onboarding.

9. Trinity MVR (Motor Vehicle Report)

   • Must be dated within 30 days of start.

10. 2 Step PPD OR IGRA (Quantiferon/Tspot)

    • Required for onboarding.

11. COVID-19 Vaccine

    • Proof of vaccine, declination, or exemption required.

12. MMR Vaccine

    • Proof of vaccination required.

13. Hepatitis B Vaccine

    • Must have 1st vaccination prior to start; proof required.

14. Influenza Vaccine

    • Proof of vaccination required for the specified flu season.

15. Varicella Vaccine

    • Proof of vaccination required.

16. Tdap Vaccine

    • Current, non-expired vaccine required.

17. Physical/Health Statement

    • Documentation from an exam within the last year required.

18. Trinity Profile

    • Resume, Skills Checklist within a year, and 2 Professional References required.

19. Trinity Conflict Of Interest Disclosure Form

    • Must have page 1 signed form on file.

20. Trinity Criminal Background Check

    • Must be dated within 30 days of start.

21. Trinity Education Verification

    • Attach completed report of highest level of academic education.

22. Trinity EPLS

    • Attach scanned EPLS report dated within the past 30 days.

23. Trinity GSA/OIG

    • Attach scanned OIG result print-out dated within the past 30 days.

24. Trinity NSO

    • Must be dated within 30 days of start.

25. Trinity System Access Form

    • Must be completed and uploaded as soon as possible.
