Job Description

• This position involves providing in-home visits for qualifying clients in the coverage area of Hampton, Newport News, Seaford, and Yorktown.
• The role requires seeing 30 points in a week or 6 visits a day, completing documentation on the same day, and working every third weekend as part of the weekend rotation.

Requirements

• Experience:

  • 2 years of Home Health experience required
  • Oasis Charting experience
  • 1st time applicants are welcome
• Licensure:

  • Virginia RN License or compact license must be in hand

Shift Information

• Every third weekend
• No call required

Skills and Competencies

• Must have Home Health experience
• Proficient in OASIS E charting
• Skills required include:

  • Wound care (including wound vacs)
  • Plurex and Central Lines
  • PICC line management
  • Lab draws
  • Comprehensive patient assessments
  • Education on Chronic Disease Management (COPD, CHF, DM, Pneumonia)

Additional Information

• Contract Length: 13 weeks
• Documentation Requirements:

  • I-9 and E-Verify
  • Government-issued photo ID and valid driver’s license
  • Various certifications (ACLS, BLS, PALS, NRP) may be required based on unit description
  • Completion of health documents including TB screening, physical exam, and vaccination records
  • Background check and references required
