Job Description

• This position is 100% field-based, with nurses traveling from one patient to the next.
• No office reporting; all meetings are held virtually.
• Initial onboarding is required on-site in Milwaukee, WI for 1-2 days.
• Case Managers can expect to see 20-30 patients per week, with an expectation of 6 routine visits per day.
• Patients typically have multiple comorbidities, including but not limited to:

  • Congestive Heart Failure (CHF)
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Pneumonia
  • Cancer
  • Dementia
  • Kidney disease
  • Hypertension
  • Falls and fractures
  • Infections and cellulitis
  • Post-surgical care
  • Wound care
  • Ostomy care
  • Labs and IV infusion

Responsibilities

• Provide education on medications, disease management, and self-care.
• Perform nursing skills such as wound care, infusion, line maintenance, and ostomy care.
• Teach patients how to perform their own care.
• Utilize EPIC for computer charting and Sapphire IV pumps.
• A laptop will be provided for charting.
• Candidates must wear solid color scrubs.
• Transportation to/from all patient visits is required.

Qualifications

• Experience: Minimum of 1 year in Home Health RN.
• Licenses/Certifications:

  • Active Basic Life Support (BLS) certification (AHA).
  • Active RN license in Wisconsin.
• Skills:

  • Wound care experience (including wound vacs, pleural drains, and ostomy care).
  • Ability to perform peripheral blood draws, central line care, and IV administration through multiple devices (elastomeric, IV pumps).
  • Excellent assessment skills.
  • Experience with computer documentation.
• Transportation: Reliable transportation, active driver’s license, and insurance are mandatory.
