• Shifts: Mostly Days, varying shifts of 8-10 hours depending on department needs.
• On Call Requirements:

  • 10-12 call shifts per month
  • Expected to be part of the on-call team during holidays.

Required Credentials and Certifications

1. Personal Health History:

   • 2 Step PPD OR IGRA (Quantiferon)
   • Trinity Specific Covid Questionnaire
   • COVID-19 Vaccine (proof of vaccination required)
   • MMR (proof of vaccination required)
   • Hepatitis B (proof of vaccination required)
   • Influenza Vaccine (proof of vaccination required)
   • Varicella (proof of vaccination required)
   • Tdap (proof of vaccination required)
   • Physical/Health Statement (fit to work)
   • Fit Test

2. Professional Certifications:

   • BLS (American Heart Association certification required)
   • ACLS (required for RNs in specific units)
   • NIHSS (required)
   • TNCC (optional)

3. Background Checks:

   • Criminal Background Check (Federal, Nationwide, County from last 7 years)
   • Education Verification (attach proof of highest level of education)
   • License/Certificate Verification (attach verification page dated within 30 days)
   • Trinity 9 Panel Drug Screen (completed within 30 days of start)

4. Additional Requirements:

   • Trinity Code of Conduct Form
   • Trinity Confidentiality of Patient and Proprietary Information Agreement
   • Trinity Conflict Of Interest Disclosure Form
   • Trinity System Access Form
   • Trinity Non Employee Upload Form
   • Trinity Integrity and Audit Services Form
