Job Description

As a Registered Nurse (RN), you will implement a full-time professional nursing service to patients in their homes under the supervision of the Clinical Services Supervisor. Your role will include:

Essential Functions and Responsibilities

Initiating and providing medically prescribed skilled nursing care and health counseling.
Conducting initial visits to assess patients’ physical and emotional status, needs, and resources.
Preparing nursing plans of care in accordance with the physician’s treatment plan and providing patient education.

Requirements

Experience: 2+ years of nursing experience (Required)
Driver's License: Required
Home Care Experience: Required
Skills: Infusion and wound care (Required), Oasis experience (Preferred)
Travel: Preferred

Additional Information

Case Load: Typically 7 visits per day
EMR System: Home Care Home Base
Practice: Point of Care
Dress Code: Matching scrubs
Mileage Reimbursement: $0.34 per mile
