Job Summary

The Registered Nurse in Home Health provides and directs nursing care to patients in their homes as prescribed by the physician, ensuring compliance with applicable laws, regulations, and agency policies. The RN coordinates the total plan of care with other healthcare professionals to maintain continuity of patient care.

Essential Functions

Clinical Competence

• Develops, implements, and revises the plan of care in collaboration with the physician and healthcare professionals.
• Supervises care provided by home health aides and licensed practical nurses, providing instruction and assigning tasks as per regulations.

Documentation and Care Delivery

• Delivers high-quality clinical services in accordance with the plan of care and infection control standards.
• Completes comprehensive assessments (OASIS) and medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements.

Quality

• Conducts initial and comprehensive nursing evaluations to ensure home health eligibility and medical necessity.
• Communicates effectively with agency staff, physicians, patients, and family members regarding patient care and service excellence.

Professionalism

• Maintains a positive attitude and is open to learning new skills.
• Punctual for meetings and actively participates, including video meetings.

Teamwork

• Completes tasks assigned by Clinical and Executive Directors in a timely manner.
• Assists in the orientation of new personnel and serves as a preceptor.
• Participates in performance improvement plans to enhance patient safety and outcomes.
• Flexibility in scheduling is required, including weekend rotations and potential holiday availability.

License Requirements

• Current RN licensure in the state of practice.
• Current CPR certification.
• Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
