Qualifications

• Required Certifications:

  • RDCS (Registered Diagnostic Cardiac Sonographer) or RCS (Registered Cardiovascular Sonographer) through the American Registry for Diagnostic Medical Sonography or Cardiovascular Credentialing International.
  • BLS (Basic Life Support) certification.

Additional Requirements

• Vaccination Requirements:

  • Proof of COVID-19 vaccination (including booster) or declination/exemption.
  • Proof of MMR vaccination or IgG titer.
  • Hepatitis B vaccination series (1st shot before start, 2nd shot 30 days later, 3rd shot 6 months later).
  • Influenza vaccine during the specified flu season.
  • Varicella vaccination or proof of immunity.
  • Tdap vaccination (declinations accepted if no pediatric interaction).
  • 2 Step PPD or IGRA (Quantiferon/Tspot) test.
• Health Documentation:

  • Physical/Health Statement from an exam within the last year.
  • 9 Panel Drug Screen completed within 30 days of start date.
• Professional Documentation:

  • Resume and Skills Checklist (within the last year).
  • 2 Professional References.
  • Signed Trinity Code of Conduct Form.
  • Confidentiality Agreement.
  • Conflict of Interest Disclosure Form.
  • Criminal Background Check (dated within 30 days).
  • Education Verification.
  • EPLS and GSA/OIG checks (dated within 30 days).
  • NSO report (dated within 30 days).
  • System Access Form.
  • Fit Test.
  • Annual TB Questionnaire.
  • Standard I9 completed by the agency.
  • Integrity and Audit Services Form.
  • CORE Exam within the last year.
  • Latex Allergy Questionnaire.
