Required Credentials and Certifications

1. BLS (Basic Life Support)

   • Required by MA DPH for all employees.
   • Accepted from AHA, Red Cross, CHSF, or Military.

2. ACLS (Advanced Cardiovascular Life Support)

   • Optional but preferred.

3. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start date.

4. Fit Test

   • N95 fit test required within 365 days prior to start date and updated annually.

5. MGB Nurse Aide Registry Check

   • Required within 30 days of start.

6. COVID-19 Vaccine

   • Completed primary vaccination series required; no exemptions considered.

7. MMR (Measles, Mumps, Rubella)

   • Documentation of vaccinations or proof of immunity required.

8. Hepatitis B

   • Strongly preferred to have documentation of vaccinations or proof of immunity.

9. Influenza Vaccine

   • Required during flu season (Sept 15th - April 30th); no exemptions considered.

10. TB Test

    • Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.

11. Health Statement

    • Physician statement required within 365 days of start date.

12. Tdap Vaccine

    • Preferred to have documentation within 10 years.

13. Criminal Background Check

    • Required within 45 days prior to start date.

14. Massachusetts CORI

    • Required within 30 days prior to start date.

15. Resume

    • Must include past 7 years of work experience and education.

16. References

    • Minimum of 2 recent references, including at least 1 supervisor.

17. Skills Checklist

    • Required for submission.

18. Confirmation of Assignment/POI Form

    • Required upon acceptance of assignment.

19. Education Verification

    • Verification of highest level of education achieved.

20. SSN Trace

    • Required to verify addresses and aliases for the past 7 years.

21. National Sex Offender Check

    • Required within 30 days of start and annually.

22. Exclusion Checks

    • Required to search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

23. TB Symptom Survey

    • Required if positive PPD.

24. Submission Checklist

    • Required for submission.
