Job Overview: Surgical Technician

Location: Jamaica Plain, Massachusetts

Schedule: 36-hour work week, Days

Required Credentials and Qualifications

1. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start date. MA Conditional Licenses not accepted.

2. Certified Surgical Technologist (CST)

   • Required for onboarding.

3. COVID-19 Vaccine

   • Completed primary vaccination series or up-to-date COVID vaccine required. No exemptions or waivers considered.

4. MMR Vaccination

   • Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity via blood test required.

5. Influenza Vaccine

   • Required during flu season (Sept 15 - April 30). No exemptions or waivers considered.

6. TB Test

   • Documentation of TB skin test screening or negative QuantiFERON TB Test required within 3 months of start date.

7. Health Statement

   • Physician statement signed by MD/DO/NP/PA required within 365 days of start date.

8. MGB Criminal Background Check

   • Required for onboarding, including a vetting process with MGH/Security.

9. MGB Massachusetts CORI

   • Criminal history check required within 30 days prior to start date.

10. MGB Resume

    • Must include past 7 years of work experience and education.

11. MGB References

    • Two recent references required, with at least one from a supervisor or above.

12. MGB Skills Checklist

    • Required for submission.

13. MGB Confirmation of Assignment/POI Form

    • Required for all initial assignments and extensions.

14. MGB Education Verification

    • Verification of highest level of education achieved required.

15. MGB SSN Trace

    • Must verify addresses/aliases for the past 7 years.

16. MGB National Sex Offender Check

    • Required within 30 days of start and annually.

17. MGB Exclusion Checks

    • Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists required.

18. Respirator Medical Clearance (RMC) Attestation

    • Required for onboarding.

19. TB Symptom Survey (if Positive PPD)

    • Must be completed prior to start and updated annually.

20. MGB Submission Checklist

    • Required for submission.

Additional Information

• Strongly preferred: Documentation of 3 Hepatitis B vaccines or proof of immunity.
• Tdap vaccine within 10 years is preferred but not required.
• All documentation must be uploaded as specified, and updates are required if there is a break in service.
