Job Opportunity: RN Telemetry I

Overview

• Contract Length: 12 weeks
• Schedule: 36 hours/week, every other weekend; will float within the med/surg/tele cluster.

Required Credentials

1. 2 Step PPD OR IGRA (Quantiferon): Required for onboarding.
2. Trinity OFAC: Must be completed within 30 days of start.
3. BLS Certification: Required for all direct patient care personnel.
4. ACLS Certification: Required for RNs in CCU, ED, PACU, MICU, SICU.
5. License/Certificate Verification: Must be dated within 30 days of start.
6. Fit Test: Required for onboarding.
7. Trinity Code of Conduct Form: Must have page 27 signed.
8. Trinity I9: Standard I9 completed by the agency.
9. Trinity Confidentiality Agreement: Must have page 5 signed.
10. Trinity Integrity and Audit Services Form: Required for onboarding.
11. Trinity Respirator Evaluation: Physician signature required.
12. Trinity Specialty Specific Competency Testing: Must be completed within a year.
13. Trinity Skills Checklist: Required for clinical contractors.
14. COVID-19 Vaccine: Proof of vaccination or declination required.
15. MMR Vaccination: Proof required.
16. Hepatitis B Vaccination: Proof required.
17. Influenza Vaccine: Proof required during specified dates.
18. Varicella Vaccination: Proof required.
19. Tdap Vaccination: Current proof required.
20. Physical/Health Statement: Documentation required.
21. Trinity 9 Panel Drug Screen: Must be completed within 30 days of start.
22. Trinity Profile: Resume, Skills Checklist, and 2 Professional References required.
23. Trinity Conflict Of Interest Disclosure Form: Must have page 1 signed.
24. Trinity Criminal Background Check: Must be dated within 30 days of start.
25. Trinity Education Verification: Verification of highest level of education required.
26. Trinity EPLS: Scanned report required.
27. Trinity GSA/OIG: Scanned report required.
28. Trinity NSO: Scanned report required.
29. Trinity System Access Form: Must be completed and uploaded as soon as possible.
