Job Opportunity: RN IR/EP

Overview

• Shift Details: 12-hour shifts (36 hours per week) or 4 ten-hour shifts (Monday to Friday)

Required Certifications and Credentials

1. BLS (Basic Life Support)

   • Certification from AHA, Red Cross, CHSF, or Military.
   • Required for onboarding.

2. ACLS (Advanced Cardiovascular Life Support)

   • Certification from AHA, Red Cross, CHSF, or Military.
   • Required for onboarding.

3. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification.
   • Required within 30 days of start date.

4. Fit Test

   • N95 fit test required within 365 days prior to start date.
   • Must be completed on the MGB N95 Respirator Fit Test Record Form.

5. MGB Medication Exam

   • 80% passing score required within 365 days for RN & LPN.

6. COVID-19 Vaccine

   • Completed primary vaccination series required; no exemptions considered.

7. MMR (Measles, Mumps, Rubella)

   • Documentation of vaccinations or proof of immunity required.

8. Influenza Vaccine

   • Required during flu season (September 15th - April 30th); no exemptions considered.

9. TB Test

   • Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.

10. Health Statement

    • Physician statement signed by MD/DO/NP/PA required within 365 days of start date.

11. MGB Criminal Background Check

    • Required within 45 days prior to start date.

12. MGB Massachusetts CORI

    • Attestation required within 30 days prior to start date.

13. MGB Resume

    • Must include past 7 years of work experience and education.

14. MGB Skills Checklist

    • Required for submission.

15. MGB Confirmation of Assignment/POI Form

    • Required upon acceptance of assignment.

16. Education Verification

    • Verification of highest level of education achieved.

17. MGB SSN Trace

    • Required for background check.

18. MGB National Sex Offender Check

    • Required within 30 days of start.

19. MGB Exclusion Checks

    • Required for compliance with OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

20. MGB TB Symptom Survey (if Positive PPD)

    • Required prior to start and updated annually.
