Overview

Job Description

The Registered Nurse will coordinate and direct home nursing care according to physician orders, agency policies, and procedures. Responsibilities include:

• Supervising and evaluating Home Health Aides and Licensed Practical Nurses.
• Conducting home visits and participating in periodic office meetings.
• Managing an average of 5-8 patients per day, with a productivity expectation of 30 points per week.

Requirements

• Experience: Minimum of 1 year of Adult Home Health or Hospice experience.
• Certifications Required:

  • CPR
  • BLS
• On-Call Requirement: Varies by location; to be discussed during the interview.
• Travel: Must be willing to travel up to 75 miles from the home clinic.
• Local Candidates: Accepted at the same rate.

Work Schedule

• Typical Hours: Monday to Friday, 8 AM to 5 PM
• Weekend Requirement: Varies by location; to be discussed during the interview.
• On-Call Requirement: Varies by location; to be discussed during the interview.

Additional Information

• Charting System: Proficiency in HomeCare HomeBase (HCHB) is required; recent experience within the last 6 months is preferred.
• Types of Cases Managed: Start of Care, Wound Care, Follow-up Visits, etc.
• Dress Code: Scrubs

Compliance and Documentation

Candidates must complete various compliance documents and training prior to starting, including:

• Acknowledgment of the Code of Conduct
• Compliance Training Certification
• HIPAA and Privacy Policy review
• Background checks and health documentation (e.g., TB test, vaccinations)
