Job Overview

Position: RN Critical Care
Location: Ravenna, Ohio
Facility: Portage Medical Center, University Hospitals
Employment Type: Travel
Contract Length: 13 weeks

Required Credentials

1. BLS (Basic Life Support)

   • Certification from American Heart Association required.

2. ACLS (Advanced Cardiovascular Life Support)

   • Certification from American Heart Association required.

3. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required within 30 days of start. Must have a valid permanent RN license.

4. MMR (Measles, Mumps, Rubella)

   • Positive titers ONLY from within the last 15 years. Specific booster requirements apply.

5. Fit Test

   • Completed within 1 year of start. Specific mask types listed.

6. Hepatitis B

   • Proof of full series or at least two vaccines required, or signed declination.

7. Varicella (Chickenpox)

   • Positive titers ONLY from within the last 15 years. Specific booster requirements apply.

8. Tdap (Tetanus, Diphtheria, Pertussis)

   • Proof of vaccination within the past 10 years required.

9. 2-Step PPD or IGRA (Quantiferon/Tspot)

   • Negative reads required within the last 6 months.

10. NIHSS (National Institutes of Health Stroke Scale)

    • Certification required.

11. Physical/Health Statement

    • Completed within one year of initial start date, stating ability to work without restrictions.

12. UHS I-9 Attestation

    • Completed I-9 or attestation required.

13. UHS Education Verification

    • Highest level of education verified; attestations not allowed.

14. UHS Employment Verification

    • Verification of employment within the last 5 years required.

15. UHS Criminal Background Check Results

    • Background check results required within 6 months of start.

16. UHS OIG Search

    • Must be completed within 6 months prior to start.

17. UHS Drug Screen

    • Must be completed within 60 days of initial start date.

18. UHS Module Completion Certificate

    • Safety and Compliance Training modules must be completed prior to clearance.

Additional Information

• COVID-19 Vaccine: Proof of vaccination preferred but not required.
• Annual Negative PPD: If not done within the past year, required.
• Resume and References: Resume to include education and work history, with 2 professional references required within the last 2 years.
• Badge Photo: A headshot with a plain background required.
