Position: RN Critical Care
Location: Ravenna, Ohio
Facility: Portage Medical Center, University Hospitals
Employment Type: Travel

Required Credentials and Certifications

1. BLS (Basic Life Support)

   • Required for clinical positions.
   • Certification from the American Heart Association only.

2. ACLS (Advanced Cardiovascular Life Support)

   • Certification from the American Heart Association only.

3. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification.
   • Must have a valid permanent RN license; temporary licenses are not accepted.

4. MMR (Measles, Mumps, Rubella)

   • Positive titers only from within the last 15 years.
   • Specific booster requirements apply.

5. Fit Test

   • Must be completed within 1 year prior to start.

6. Hepatitis B

   • Proof of full series or at least two vaccines from the series, or signed declination.

7. Varicella (Chickenpox)

   • Positive titers only from within the last 15 years.
   • Specific booster requirements apply.

8. Tdap (Tetanus, Diphtheria, Pertussis)

   • Proof of vaccination within the past 10 years.

9. 2-Step PPD or IGRA (Quantiferon/Tspot)

   • Negative reads within the last 6 months.

10. NIHSS (National Institutes of Health Stroke Scale)

    • Certification required.

11. UHS I-9 Attestation

    • Completed I-9 or attestation that completed I-9 is on file.

12. UHS Criminal Background Check Results

    • Must be completed within 6 months of start.

13. UHS Education Verification

    • Highest level of education verified; attestations not allowed.

14. UHS Employment Verification

    • Verification of employment within the last 5 years.

15. UHS Skills Checklist

    • Skill-specific checklist required to be completed within 1 year prior to start.

16. UHS Competency Exam

    • Must score at least 80%; to be completed within 1 year prior to start.

17. UHS Core Competencies

    • 80% passing score required; completed within 1 year prior to start.

18. Physical/Health Statement

    • Pre-employment exam statement signed, stating the traveler is able to work without restrictions.

19. Influenza Vaccine

    • Proof of vaccination from October to March or approved exemptions.

20. UHS Module Completion Certificate

    • Completion of safety and compliance training modules.
