Job Opportunity: RN Step Down

Location

Required Credentials and Certifications

  1. BLS (Basic Life Support)

    • Required for clinical positions.
    • American Heart Association certification only.
  2. ACLS (Advanced Cardiovascular Life Support)

    • American Heart Association certification only.
  3. License/Certificate Verification

    • Primary Source Verification of professional license and/or certification required within 30 days of start and updated upon expiration. Temporary licenses are NOT accepted.
  4. MMR (Measles, Mumps, Rubella)

    • Positive titers ONLY from within the last 15 years. Specific booster requirements apply.
  5. Fit Test

    • Completed within 1 year of start, updated annually.
  6. Hepatitis B

    • Proof of full series or at least two vaccines from the series, or signed declination.
  7. Varicella

    • Positive titers ONLY from within the last 15 years. Specific booster requirements apply.
  8. Tdap (Tetanus, Diphtheria, Pertussis)

    • Proof of vaccination within the past 10 years.
  9. 2-Step PPD or IGRA (Quantiferon/Tspot)

    • Negative reads within the last 6 months. If positive, a clear chest x-ray and current TB questionnaire are required.
  10. NIHSS (National Institutes of Health Stroke Scale)

    • Required for clinical positions.
  11. UHS I-9 Attestation

    • Completed I-9 or attestation that completed I-9 is on file.
  12. UHS Education Verification

    • Highest level of education verified. Attestations not allowed.
  13. UHS Employment Verification

    • Verification of employment within the last 5 years.
  14. UHS Skills Checklist

    • Clinical only, required to be completed within 1 year prior to start.
  15. UHS Competency Exam

    • Must score at least 80%, completed within 1 year prior to start.
  16. UHS Core Competencies

    • 80% passing score required, completed within 1 year prior to start.
  17. Physical/Health Statement

    • Completed within one year of initial start date, stating the traveler is able to work without restrictions.
  18. Influenza Vaccine

    • Proof of vaccination from October to March or approved exemptions.
  19. UHS Criminal Background Check Results

    • Ran within 6 months of start.
  20. UHS OIG Search

    • Verified to be ran separately from background check.
  21. UHS SAM/EPLS/GSA/OFAC

    • Supporting documentation that the individual was screened and cleared.
  22. UHS Drug Screen

    • Ran within 60 days upon initial start date.
  23. UHS Module Completion Certificate

    • Safety and Compliance Training modules to be completed prior to clearance.
  24. UHS National Database Criminal Background Check

    • For Ohio residents to meet fingerprint requirements.

Additional Information

  • COVID-19 Vaccine: Proof of vaccine series or completion of medical/religious exemption form preferred but not required.
  • Annual Negative PPD: If 2-step PPD not done within the past year.
  • Resume and References: Resume to include education and work history, with 2 professional references required within the last 2 years.
