Job Description

• Required certifications: ACLS, BLS, and NIHSS.
• Must have Epic experience within the last year.
• Floating to other units is required.
• Solid black scrubs are mandatory.
• Phone interviews are required; candidates should provide their availability.
• Graduate of an accredited school of professional nursing is required.

Required Credentials

Personal Health History

• COVID-19 Vaccine: Fully vaccinated (2 doses if applicable). Medical or religious exemptions considered on a case-by-case basis.
• MMR: Titer or 2 adult MMR vaccines required. Negative titer requires two boosters.
• Hepatitis B: Titer or proof of complete vaccines required. Negative titer will require 2 boosters.
• Influenza Vaccine: Required by all travelers from September to April. Must be completed by 11/7/23.
• PPD or IGRA: 2 Negative PPDs or negative Q-Gold/Tspot required.
• Varicella: Titer or 2 adult vaccines required. Negative titer requires 2 boosters.
• Tdap: Must be administered within 10 years of start.
• Annual TB Questionnaire: Completed, signed, and dated.
• Drug Screen: Negative 10 panel drug screen within 30 days of start.
• Fit Test: Required for all patient-facing positions.
• Health Statement: Pre-employment physical required within the year of start.

Professional Credentials

• BLS Certification: Required for all nurses.
• ACLS Certification: Required.
• Nursing License: Must be current and valid in Ohio.
• License/Certificate Verification: Required for state-specific registry.
• Criminal Background Check: Must be completed within 90 days of start.
• OIG and EPLS Searches: Required within 90 days of start.
• Education Verification: Required.
• Fingerprint Clearance Attestation: Required within 12 months of start.
• Social Security Card: Must provide.
• Summa Memo of Understanding: Required for specific units.

Testing and Competency

• Unit Specific Competency Exam: Required.
• Skills Checklist: Completed within the year of start and updated annually.
• Core Competencies: HIPAA, Joint Commission - NPSG, OSHA topics required within 90 days of start.

Additional Information

• RTO (Requested Time Off) must be submitted at the time of application, with a maximum of 7 days allowed.
• No block-scheduling requests will be accepted.
• Candidates will be automatically rejected if more than 7 days of RTO are listed.
