Job Description

Candidates must have NBSTA certification unless they can provide proof of CST employment since 2012.

Required Credentials and Documentation

1. BLS Certification: AHA, Red Cross, CHSF, or Military accepted.
2. License/Certificate Verification: Primary Source Verification of professional license and/or certification required within 30 days of start date.
3. COVID-19 Vaccine: Completed primary series or up-to-date vaccination required; no exemptions considered.
4. MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity via blood test.
5. Hepatitis B: Record of 3 vaccines or proof of immunity via blood test (preferred).
6. Influenza Vaccine: Required during flu season (Sept 15 - April 30); no exemptions considered.
7. TB Test: Documentation of TB skin test or negative QuantiFERON TB Test within 3 months of start date.
8. Health Statement: Signed by MD/DO/NP/PA within 365 days of start date.
9. Tdap Vaccine: Preferred if administered within the last 10 years.
10. Criminal Background Check: Must include all counties resided, employed, and attended school within the last 7 years.
11. Massachusetts CORI: Background check for criminal history in MA.
12. Resume: Include past 7 years of work experience with city & state.
13. References: Two recent references, including at least one supervisor.
14. Skills Checklist: Required for submission.
15. Confirmation of Assignment/POI Form: Required upon acceptance of assignment.
16. Education Verification: Must verify highest level of education achieved.
17. SSN Trace: Must list addresses/aliases for the past 7 years.
18. National Sex Offender Check: Conducted within 30 days of start.
19. Exclusion Checks: Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.
20. TB Symptom Survey: To be completed prior to start and updated annually.
21. Submission Checklist: Required for submission.
22. Fit Test: N95 fit test required within 365 days of start date.
