Job Description

As a Speech Language Pathologist, you will provide speech therapy services to patients in their homes who have impairments, functional limitations, or disabilities due to injury, disease, or other causes. You will work with a diverse patient population, ranging from infants to the elderly, and collaborate with patients, families, physicians, and other health professionals to identify needs and coordinate services.

Key Responsibilities

• Conduct age-appropriate evaluations reflecting patient history and relevant tests.
• Reassess treatment plans every two weeks and modify as necessary.
• Ensure physician orders are current and consistent with the plan of care.
• Manage patient schedules and documentation according to agency standards.
• Supervise home health aides in accordance with agency and state guidelines.
• Maintain communication with the interdisciplinary team regarding patient progress and goals.

Qualifications

• Licensure: Registered with the Board of Registration in Massachusetts.
• Experience: Minimum of two years in acute care, rehab, or homecare as a Speech Therapist.
• Skills:

  • Proficient computer skills and ability to learn new software.
  • Strong verbal, written, and electronic communication skills.
  • Excellent interpersonal and customer service skills.

Required Credentials

• Certifications:

  • BLS (Basic Life Support)
  • License/Certificate Verification
• Health Requirements:

  • COVID-19 vaccination
  • MMR vaccination or proof of immunity
  • TB screening
  • Influenza vaccine during flu season
• Background Checks:

  • Criminal background check
  • National sex offender check
  • Exclusion checks for Medicaid and other programs

Additional Information

• Weekend rotations are required once every six weeks.
• Documentation and verification of credentials must be completed prior to the start date.
• Strongly preferred: Documentation of Hepatitis B vaccination or proof of immunity.
