Job Description

The role involves providing acute care in a medical/surgical setting. Candidates must be fully vaccinated against COVID-19 and have the current flu vaccine.

Required Credentials and Health History

1. BLS Certification: AHA, Red Cross, CHSF, or Military accepted.
2. License/Certificate Verification: Primary source verification required within 30 days of start date. MA Conditional Licenses not accepted.
3. MGB MVH Housing Agreement: Required if moving into shared housing.
4. COVID-19 Vaccine: Completed primary series or up-to-date vaccine required; no exemptions considered.
5. MMR Vaccination: Documentation of two measles, two mumps, and one rubella vaccine or proof of immunity required.
6. Hepatitis B: Strongly preferred documentation of vaccination or proof of immunity.
7. Influenza Vaccine: Required during flu season (Sept 15 - April 30); no exemptions considered.
8. TB Test: Documentation of TB skin test or negative QuantiFERON TB Test required within 3 months of start date.
9. Health Statement: Signed by MD/DO/NP/PA within 365 days of start date.
10. Criminal Background Check: Required, including a Massachusetts CORI check.
11. Resume: Must include past 7 years of work experience with city and state.
12. References: Two recent references, including at least one supervisor.
13. Skills Checklist: Required for submission.
14. Confirmation of Assignment/POI Form: Required upon acceptance of assignment.
15. Education Verification: Must verify highest level of education achieved.
16. SSN Trace: Required to list addresses/aliases for the past 7 years.
17. National Sex Offender Check: Required within 30 days of start.
18. Exclusion Checks: Required for OIG, GSA, and MassHealth & NH Medicaid exclusion lists.
19. Fit Test: N95 fit test required within 365 days of start date.
