Massachusetts state nursing license
BLS certification required

Documentation of TB skin test or QuantiFERON TB Test required
Health Statement signed by MD/DO/NP/PA required
Documentation of flu vaccination required during flu season
MGB Criminal Background Check required
MGB Massachusetts CORI required
MGB Resume required for submission
Respirator Medical Clearance (RMC) Attestation required
