Position: RN Medical/Surgical

Required Credentials and Documentation

Certifications

• BLS (Basic Life Support)

  • Accepted: AHA, Red Cross, CHSF, or Military certifications.

Licenses

• License/Certificate Verification

  • Primary Source Verification of professional license and/or certification is required within 30 days of start date. MA Conditional Licenses are not accepted.

Health Requirements

• COVID-19 Vaccine

  • A completed primary vaccination series approved by the FDA or WHO is required. No exemptions or waivers will be considered.

• MMR (Measles, Mumps, Rubella)

  • Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine, or proof of immunity via blood test.

• Hepatitis B

  • Record of 3 Hep B vaccines or proof of immunity via blood test is strongly preferred.

• Influenza Vaccine

  • Required during flu season (Sept 15th - April 30th). No exemptions or waivers will be considered.

• TB Test

  • Documentation of TB skin test screening or negative QuantiFERON TB Test within 3 months of start date.

• Health Statement

  • Physician statement signed by MD/DO/NP/PA within 365 days of start date.

• Tdap (Tetanus, Diphtheria, Pertussis)

  • Vaccine within 10 years is preferred.

Background Checks

• MGB Criminal Background Check

  • Must include all counties resided, employed, and attended school within the last 7 years.

• MGB Massachusetts CORI

  • Criminal history check for the state of MA.

• MGB National Sex Offender Check

  • Conducted within 30 days of start and updated annually.

• MGB Exclusion Checks

  • Search OIG, GSA, and MassHealth & NH Medicaid exclusion lists.

• MGB Resume

  • Include past 7 years of work experience with city & state.

• MGB References

  • 2 recent references, with a minimum of 1 from a supervisor or above.

• MGB Skills Checklist

  • Required for submission.

• MGB Confirmation of Assignment/POI Form

  • Required for all initial assignments and extensions.

• MGB Education Verification

  • Verification of highest level of education achieved.

• MGB SSN Trace

  • Must list addresses/aliases for the past 7 years.

• Fit Test

  • N95 fit test must be completed within 365 days prior to start date.

• MGB Submission Checklist

  • Use the current version found in the Document Library, required for submission.
