Required Credentials and Certifications

1. BLS (Basic Life Support)

   • Required for clinical positions.

2. License/Certificate Verification

   • Primary Source Verification of professional license and/or certification required.

3. MMR (Measles, Mumps, Rubella)

   • Positive titers ONLY from within the last 15 years.

4. Fit Test

   • Completed within 1 year of start, updated annually.

5. Hepatitis B

   • Proof of full series or at least two vaccines required.

6. Varicella (Chickenpox)

   • Positive titers ONLY from within the last 15 years.

7. Tdap (Tetanus, Diphtheria, Pertussis)

   • Proof of vaccination within the past 10 years.

8. ARRT (American Registry of Radiologic Technologists)

   • Required certification.

9. 2-Step PPD or IGRA (Quantiferon/Tspot)

   • Negative reads within the last 6 months.

10. Physical/Health Statement

    • Completed within one year of initial start date.

11. UHS Skills Checklist

    • Skill specific required to be completed within 1 year prior to start.

12. UHS Competency Exam

    • Must score at least 80%, completed within 1 year prior to start.

13. UHS Core Competencies

    • 80% passing score required, completed within 1 year prior to start.

14. UHS Criminal Background Check Results

    • Ran within 6 months of start.

15. UHS OIG Search

    • Verified to be ran separately from background check.

16. UHS SAM/EPLS/GSA/OFAC

    • Supporting documentation that the individual was screened and cleared.

17. UHS Module Completion Certificate

    • Safety and Compliance Training modules to be completed prior to clearance.

Additional Information

• COVID-19 Vaccine: Proof of vaccine series or completion of medical/religious exemption form preferred.
• Driver's License/State ID: Non-expired photo ID required.
• Resume and References: Resume to include education and work history, with 2 professional references required within the last 2 years.
