  • 6:45 AM - 3:15 PM OR
  • 6:45 AM - 5:15 PM with call rotation

Required Certifications and Credentials

1. BLS Certification

   • American Heart Association certification required for all direct patient care personnel.
   • Attach scanned copy of certification (with expiration date).

2. License/Certificate Verification

   • Must be dated within 30 days of start.
   • Attach scanned online verification page.

3. Fit Test

   • Required prior to start for specific Iowa locations.

4. Certified Surgical Technologist (CST)

   • Required for onboarding.

5. Trinity 9 Panel Drug Screen

   • Must be completed within 30 days of start date.

6. Trinity Code of Conduct Form

   • Must have page 27 signed form on file.

7. Trinity I9

   • Standard I9 completed by the agency.

8. Trinity Confidentiality Agreement

   • Must have page 5 signed form on file.

9. Physical/Health Statement

   • Provide documentation from an exam within the last year.

10. Trinity Profile

    • Resume, Skills Checklist within a year, and 2 Professional References required.

11. Criminal Background Check

    • Federal, Nationwide, County Criminal from last 7 years required.

12. Education Verification

    • Attach completed report of highest level of academic education.

13. COVID-19 Vaccine

    • Proof of vaccine, declination, or exemption required.

14. Influenza Vaccine

    • Proof of vaccine required during specified dates.

15. Tdap Vaccine

    • Current, non-expired TDAP vaccine required.

16. Trinity IA Sing Report

    • Iowa's Criminal History, Sex Offender, Dependent Adult & Child Abuse Registries check required.

17. Trinity Skills Checklist

    • Required for clinical contractors.
