Required Credentials and Certifications

1. BLS Certification

   • American Heart Association certification required for all direct patient care personnel.
   • Attach scanned copy of certification (with expiration date) to candidate's record.

2. License/Certificate Verification

   • Must be dated within 30 days of start.
   • Attach scanned online verification page with a machine-generated date indication.

3. Fit Test

   • Required for onboarding.

4. Trinity 9 Panel Drug Screen

   • Must be completed within 30 days of start date, preferred within 14 days.
   • Must include specific substances (e.g., Cocaine, Amphetamines, Opiates, etc.).

5. Trinity Code of Conduct Form

   • Must have page 27 signed form on file.

6. Trinity I9

   • Standard I9 completed by the agency.

7. Trinity Confidentiality Agreement

   • Must have page 5 signed form on file.

8. Trinity Integrity and Audit Services Form

   • Required for onboarding.

9. Trinity Specialty Specific Competency Testing

   • Attach copy of specialty exam completed within a year.

10. COVID-19 Vaccine

    • Proof of vaccine required, declination or exemption accepted with agency letterhead.

11. MMR Vaccination

    • Attach scanned proof of 2 vaccinations with dates.

12. Hepatitis B Vaccination

    • Must have 1st vaccination in series prior to start.

13. Influenza Vaccine

    • Proof of vaccine required, declination or exemption accepted with agency letterhead.

14. Varicella Vaccination

    • Attach scanned proof of 1 vaccination with date.

15. Tdap Vaccination

    • Upload a current, non-expired TDAP vaccine.

16. Physical/Health Statement

    • Provide documentation from an exam within the last year.

17. Trinity Profile

    • Resume, Skills Checklist within a year, and 2 Professional References required.

18. Trinity Conflict Of Interest Disclosure Form

    • Must have page 1 signed form on file.

19. Trinity Criminal Background Check

    • Must be dated within 30 days of start.

20. Trinity Education Verification

    • Attach completed report of highest level of academic education.

21. Trinity EPLS

    • Attach scanned EPLS report dated within past 30 days.

22. Trinity GSA/OIG

    • Attach scanned OIG result print-out dated within past 30 days.

23. Trinity NSO

    • Must be dated within 30 days of start.

24. Trinity System Access Form

    • Must be completed and uploaded as soon as possible.

25. 2 Step PPD OR IGRA (Quantiferon)

    • Must be completed within 30 days of start.

26. Trinity OFAC

    • Must be completed within 30 days of start.
