MRI Technician Position Overview

Location: Pontiac, Michigan
Facility: Trinity Health Oakland
Hospital System: Trinity Health
Employment Type: LTA (Long-Term Assignment)
Contract Length: 12 months
Shift: 1400-0030 and every other weekend

Job Description

The position is for an MRI Technician, requiring AART-MRI certification.

Required Credentials and Documentation

Personal Health History

Fit Test: Required for onboarding.
2 Step PPD OR IGRA (Quantiferon/Tspot): Required for onboarding.
Trinity OFAC: Must be completed within 30 days of start.
Trinity Specific Covid Questionnaire: Required for onboarding.
Trinity Respirator Evaluation: Physician signature required.
COVID-19 Vaccine: Proof of vaccination, declination, or exemption required.
MMR: Proof of vaccination required.
Hepatitis B: Proof of vaccination required.
Influenza Vaccine: Proof of vaccination required.
Varicella: Proof of vaccination required.
Tdap: Current, non-expired vaccine required.
Physical/Health Statement: Documentation from an exam within the last year required.

Certifications

BLS (Basic Life Support): American Heart Association certification required.
License/Certificate Verification: Must be dated within 30 days of start.

Professional Documentation

Trinity Profile: Resume, Skills Checklist (within a year), and 2 Professional References required for submission.
Trinity Conflict Of Interest Disclosure Form: Required for onboarding.
Trinity Criminal Background Check: Must be dated within 30 days of start.
Trinity Education Verification: Verification of highest level of academic education required.
Trinity EPLS: Must be dated within the past 30 days.
Trinity GSA/OIG: Must be dated within the past 30 days.
Trinity NSO: Must be dated within the past 30 days.
Trinity System Access Form: Must be completed and uploaded as soon as possible.

Additional Information

All engaged contractors must have specific forms signed and on file in Trinity VMS.
A 9 Panel Drug Screen must be completed within 30 days of start date, preferred within 14 days.

This position requires a comprehensive set of credentials and documentation to ensure compliance with healthcare regulations and standards.
