Fit Test: Personal Health History
2 Step PPD OR IGRA (Quantiferon/Tspot): Personal Health History
Trinity OFAC: Professional
Trinity Specific Covid Questionnaire: Personal Health History
Trinity Respirator Evaluation: Personal Health History
BLS (Basic Life Support): Certification required for all direct patient care personnel.
License/Certificate Verification: Must be dated within 30 days of start.
Trinity 9 Panel Drug Screen: Must be completed within 30 days of start date.
Trinity Code of Conduct Form: Professional
Trinity I9: Professional
Trinity Confidentiality of Patient and Proprietary Information Agreement: Professional
Trinity Integrity and Audit Services Form: Professional
Trinity Specialty Specific Competency Testing: Checklist/Testing
COVID-19 Vaccine: Proof of vaccination required.
MMR (Measles, Mumps, Rubella): Proof of vaccination required.

Trinity Profile: Resume, Skills Checklist within a year, and 2 Professional References.
Trinity Conflict Of Interest Disclosure Form: Professional
Trinity Criminal Background Check: Must be dated within 30 days of start.
Trinity Education Verification: Verification of highest level of academic education.
Trinity EPLS: Must be dated within the past 30 days.
Trinity GSA/OIG: Must be dated within the past 30 days.
Trinity NSO: Must be dated within 30 days of start.
Trinity System Access Form: Must be completed and uploaded as soon as possible.

All engaged contractors must have specific forms signed and on file in Trinity VMS.
