Job details

Provides coordinates and directs the provision of home nursing care according to physicians orders based on agency policies and procedures through the competent application of the nursing process. Responsible to Clinical Manager Directly Supervises and Evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses.
Home visits. Periodic office meetings. Must have reliable transportation current drivers license and agency-required liability insurance.
Average # of patients/day: 5-8
Types of Cases: Start of Care Wound Care Follow up Visits etc.
Years of experience REQ: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty.
First-timers accepted: (Y/N) No must have recent specialty experience.
Hospital/Agency Highlights Dress Code: Scrubs
Charting: All professions must be proficient with HomeCare HomeBase (HCHB) and used it within the last 6 months.

Job requirements

Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience depending on the specialty.

Schedule information

Most will be standard 5x8s M-F.
Weekend REQ: Varies per location and profession please have traveler discuss during the interview.
Will there be any weekend rotation If so how often Once a month.

Mileage reimbursed IRS Standard rate.
Locals accepted: (Y/N) Yes local accepted at the same rate.
