Description
Klarus Home Health / Jet Health, Inc. is seeking a compassionate and skilled Registered Nurses to join our team full time, part time or PRN / Per diem completing admissions for our new Home Health patients on the weekends in the Denver, CO and surrounding areas.

The primary functions of this role are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family, and referring agency, and assume the responsibility for coordination of care.

Requirements
Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
Licensure: Current state license as a Registered Nurse. Proof of current CPR certification. Current Driver's License.
Experience: Two years’ experience as a Registered Nurse in a clinical setting and one-year full-time home health experience preferred but not required.
Knowledge of OASIS highly preferred but not required
Skills: Strong assessment and clinical skills, excellent communication skills, ability to work independently and as part of a team. Familiarity with infection control measures (OSHA). Knowledge of State and Federal regulations. Good interpersonal skills.
Home Care Home Base (HCHB) knowledge highly preferred
Works in patients' homes in various conditions and locations.
Assess home health patients utilizing the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
Ability to work a flexible schedule; ability to travel locally
PRN emergency call availability.
Develop and document the plan of care in correlation with the assessment.
Provide nursing care utilizing infection control measures (OSHA).
Work with the Interdisciplinary Group to ensure patient/family needs are met.
Communicate with attending physicians, home health physicians, and other physicians involved in the patient’s care.
Timely submission of all required paperwork.
Demonstrate commitment, professional growth, and competency.
Meet productivity standards.
Perform on-call responsibilities and provide on-call service to patients/families as assigned.
Provide effective communication to patient/family, team members, and other healthcare professionals.
Transportation: Reliable transportation and valid driver's license. Valid and current auto liability insurance.
