Description
First Choice Home Health is seeking a Licensed Physical Therapy Assistant to join our team on a part-time, or PRN / per diem basis. The successful candidate will work under the supervision of a Licensed Physical Therapist to provide comprehensive physical therapy services to our patients currently enrolled in our home health services in the Boise, ID and surrounding areas!

Requirements
Key Responsibilities:

Assist in evaluating patients' physical and functional abilities under the supervision of a Licensed Physical Therapist.
Implement and modify physical therapy treatment programs based on patients' individualized care plans.
Provide hands-on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and achieve their rehabilitation goals.
Educate patients and their families on therapeutic exercises, proper body mechanics, and injury prevention techniques.
Participate in the coordination of care with interdisciplinary healthcare teams to ensure holistic patient care.
Maintain accurate and timely documentation of patient progress, treatment interventions, and communication with the supervising Physical Therapist.
Adhere to therapy practice acts and regulatory agency guidelines in the delivery of physical therapy services.
Requirements:

Graduate of an accredited school of Physical Therapy Assistant with an Associate Degree in Physical Therapy.
Current State License or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC), if practicing in a state member of the PTLC.
Minimum of one year experience as a Physical Therapy Assistant, preferred.
Proficient therapy skills as outlined in the Therapy Practice Act and regulatory agencies.
Good interpersonal skills and ability to work effectively in a team environment.
Proof of current CPR certification.
Reliable transportation, current driver's license, and valid auto liability insurance.
Ability to work a flexible schedule and travel locally as needed.
Ability to perform physical tasks associated with patient care, including heavy lifting, bending, pulling, pushing, and standing for prolonged periods.
