Description
The University of Miami/UHealth Department of Neurology has an exciting opportunity for a Full Time Social Worker to work at the UHealth Medical Campus.
Core Job Summary
The Social Worker, MSW provides social services to patients and their families, including counseling and support. Moreover, this position establishes a plan of care that fosters personal worth, spiritual well-being, and human dignity in each patient.
Core Responsibilities
Collaborates and facilitates patient discharge plans for those requiring post inpatient care.
Reassesses plan of care according to the physical and emotional needs of patient.
Performs patient assessment, including looking at illness, mental status, and family dynamics.
Documents relevant data accurately and in compliance with laws and regulations.
Links patients and their families to appropriate community resources.
Serves as patient advocate and liaison to physicians, insurance companies, and community agencies.
Helps the patient and their family to understand, accept, and follow medical recommendations.
Provides clinical supervision and guidance to junior social workers.
Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions
Responsible for social work services at the multidisciplinary neurodegenerative disease clinic and research center.
Provides Advanced Care Planning for Clinic as well as bio psycho-social assessment for clinic patients and their families.
Assist LCSW with clinical services for clinic and research patients.
Supports our education and outreach department at community events.
Provides information and referral to public or private agencies with appropriate follow-up.
Provides information to patients about appropriate insurances and disability process.
Provides information and referrals of other agencies that may offer services for patients with LBD, AD, FTD .
Provide brief support and counseling to patients and families to help adjust and adapt to a changed lifestyle.
Identify patient abuse or neglect and make contact with state investigating agency when appropriate- reports abuse as identified.
Identify environmental impediments to patient progress through interview and review of patient records.
Identify and develop support programing for clinic patients and their families including but not limited to support groups.
Provide support to our research team including brief counseling support to participants as they adjust to diagnosis.
Maintains confidentiality of all patients’ information according to HIPPA guidelines from University of Miami.

Core Qualifications
Master’s degree in relevant field
Minimum 1 year of experience
Knowledge, Skills and Attitudes:
Ability to work independently while supervised by an LCSW as well as work collaboratively with research team.
Ability to communicate effectively in both oral and written form.
Ability to be trained on and navigate EHR system.
Ability to handle difficult and stressful situations with professional composure.
Ability to maintain effective interpersonal relationships.
