Description

Registered Nurse Specialty, Stem Cell Transplant - Intensive Care Unit, Full Time

Responsible for the delivery of patient care through the nursing process of assessment, diagnosing, planning, implementation, and evaluation. Responsible for directing and coordinating all nursing care for critically ill patients based on established clinical nursing practice standards. Collaborates with other professional disciplines to ensure effective and efficient patient care delivery and the achievement of desired patient outcomes. Serves in the Resource Nurse role as directed. Supports the organizations vision and mission. Utilizes knowledge of patient’s age and cultural diversity in the provision of patient care. Contributes to the provision of quality nursing care through performance improvement techniques that demonstrate positive outcomes in patient care.

Accountabilities:

Assessment
1. Initiates assessment of vital signs, orientation to room, allergies, and notification of physician within the timeframe stipulated in unit policy.
2. Integrates all information available and completes initial patient systems assessment within the time frame established by unit policy.
3. Recognizes and responds to diagnostic data and initiates appropriate action or intervention as evidenced by documentation reviewed by management
4. Anticipates and promotes patient progress through early and accurate assessment.
5. Collects pertinent data using appropriate assessment techniques
6. Demonstrates ability to provide safe and competent care for patient undergoing invasive hemodynamic monitoring
7. Able to provide safe nursing care as per standards of care for patients requiring mechanical ventilation
8. Determines the priority of data collection based on the patient’s immediate condition or needs.
9. Involves the patient, significant others, and health care providers in data collection when appropriate.
10. Ensures that the data collection process is systematic and ongoing to identify progress and trends that require intervention.
11. Anticipates and promotes patient progress through early and accurate assessment.

Outcome Identification
1. Identifies expected outcomes individualized to the patient.
2. Derives outcomes from the diagnoses/patient problems
3. Outcomes are document in terms of measurable goals
4. Formulates outcomes mutually with the patient and healthcare providers, when possible
5. Ensures outcomes are realistic in relation to resources available to the patient

Planning
1. Initiates a written nursing plan of care upon admission
2. Develops a plan of care that prescribes interventions to attain expected outcomes
3. Reviews and evaluates the plan of care, updates, and documents patient's progress toward resolution of problems on every shift as evidenced by chart review.
4. Develops discharge plan with the physician, patient, and/or family and other available resources during the hospital stay.
5. Ensures that the plan of care reflects current nursing practice
6. Documents legibly pertinent information in the patients record

Implementation
1. Administers medications within stipulated time frame, observes effects, recognizes and reports adverse reactions
2. Initiates, administers and observes intravenous therapy according to hospital policy and procedure throughout the shift.
3. Demonstrates the ability to prepare, titrate and monitor vasoactive drugs
4. Assists physicians with the insertion of invasive monitoring equipment, obtains readings, documents and determines significance of readings.
5. Provides individualized instructions to patient and/or significant others regarding the plan of care on an on-going basis with written documentation of patient's and/or significant other's ability to verbalize
Understanding of information taught.
6. Coordinates and implements prescribed procedures to ensure accuracy, timeliness, and patient comfort.
7. Implements nursing orders autonomously on a daily basis
8. Provides psychological support and motivation to enable patients to function at their optimal level of independence as observed and documented.
9. Sets priorities for direct patient care as well as assisting other nurses in establishing priorities.

Evaluation
1. Determines level of accomplishment of expected outcomes on a daily basis as evidenced by patient progress record and plan of care.
2. Readjusts plan of care based on evaluation of the patients’/families’ needs.
3. Recognizes the need for aggressive discharge planning based on patient's needs as evidenced by interactions with physicians, Social Services, and other resources.
4. Evaluates the patient's care and progress through reviewing of patient progress record and plan of care
5. Ensures evaluation is systematic and ongoing.
6. Evaluates the effectiveness of interventions in relation to outcomes.
7. Utilizes ongoing assessment data to revise nursing diagnoses/patient problems, outcomes and the plan of care as needed.
8. Involves the patient, significant others, and health care providers in the evaluation process, when appropriate

1. Consistently maintain complete confidentiality of all medical, financial, employees, computer and other sensitive materials, which may violate HIPAA regulations and/or jeopardize the privacy of others.

4. Demonstrates capability to correlate theoretical knowledge with clinical practice on a daily basis.
5. Recognizes self-limitations and utilizes appropriate resources for referral of patients.

7. Demonstrates the ability to make shift assignments based on patient needs and allocated staff when assuming resource duties
8. Acts as a role model and maintain professional demeanor at all times
9. Maintains clinical competencies required to perform duties
10. Maintains all certifications set forth by institution
11. Attends meetings as required
12. Attends all mandatory in-services as directed
13. Actively participates in developing and maintaining performance improvement activities on the unit and hospital wide
14. Acquires and maintains current knowledge in nursing practice
15. Shares knowledge and skills with colleagues and others
16. Establishes good rapport and cooperative working relationships with personnel in the organization
17. Develops cooperative/therapeutic relationship with the patient and/or significant others and projects a positive cooperative and courteous image.
18. Promotes public relations by initiating positive communication with physicians and family members on a daily basis.
19. Promotes a working relationship based on open communication and an atmosphere of mutual understanding and trust.
20. Takes an active role in the unit’s function by assuming control and following through on assigned responsibilities.
21. Utilizes the nursing process to problem solve and informs the Nursing Manager or designee of outcome and/or if follow-up is required.
22. Demonstrates the ability to communicate via chain of command.

MINIMUM QUALIFICATIONS
Education:

Graduate from an accredited school of nursing, Bachelor’s degree (BSN) preferred. RNs hired with an Associates (ASN) degree have 2 years to complete the BSN degree.

Certification and Licensing:

Valid Florida Registered Nurse License, ACLS, BLS certification from the American Heart Association required.

Experience:

Minimum 2 years recent Critical Care Nursing (ICU) experience preferred

Recent experience in administering chemotherapy and biotherapy agents preferred

Strong interpersonal communication skills

Moderate computer literacy and skills
