Description

Job Title: Palliative Care Registered Nurse (Palliative Care RN)** Job Summary:

A Palliative Care Registered Nurse (Palliative Care RN) provides specialized care to patients with serious, chronic, or life-limiting illnesses, focusing on relieving symptoms, managing pain, and improving quality of life. The Palliative Care RN works collaboratively with patients, families, and interdisciplinary teams to develop individualized care plans that address physical, emotional, psychological, and spiritual needs.

Key Responsibilities:
  • Assess and manage patients’ physical, emotional, and psychological symptoms, including pain, fatigue, nausea, anxiety, and depression.
  • Collaborate with interdisciplinary teams, including physicians, social workers, chaplains, and therapists, to develop and implement comprehensive palliative care plans.
  • Provide education and support to patients and families regarding illness management, treatment options, and end-of-life care decisions.
  • Advocate for patients and families, helping them navigate complex medical decisions, treatments, and care options.
  • Develop and maintain individualized care plans focused on symptom control, comfort, and quality of life.
  • Administer medications and treatments, including palliative therapies such as pain management, comfort care, and symptom control.
  • Monitor and evaluate patients’ progress, adjusting care plans as needed to meet changing needs and preferences.
  • Provide emotional support and counseling to patients and families, addressing concerns related to illness, grief, and end-of-life care.
  • Support advanced care planning, including advance directives, hospice referrals, and coordination of care across settings.
  • Educate patients, families, and caregivers about palliative care services, goals, and resources available.
  • Collaborate with other healthcare professionals to ensure continuity of care and smooth transitions between care settings.
Qualifications:
  • Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted.
  • Licensure:
    • Valid Registered Nurse (RN) license in the state of practice.
  • Certification:
    • Basic Life Support (BLS) certification required.
    • Advanced Cardiac Life Support (ACLS) preferred.
    • Certified Hospice and Palliative Nurse (CHPN) preferred.
  • Experience: Minimum of 2-3 years of experience in acute care, oncology, or hospice nursing preferred.
