Description

The Hospice Case Manager is responsible for patient assessments, care planning, and coordination of services. Key responsibilities include:
Assessments & Care Plans: Evaluate patient needs and develop/adjust care plans.
Documentation: Complete timely assessments and ensure accurate records.
Education: Provide patient/family education and document interventions.
Case Management: Prioritize care needs and collaborate with the team.
Safety & Compliance: Follow safety protocols and manage medications.
Team Support: Participate in meetings and mentor new staff.
Qualifications: RN or LCSW with hospice experience preferred. Strong communication and case management skills.
Skills:
Must have 2-3 years as RN in home hospice environment
Must have valid New Jersey driver’s license
