Description

Key Responsibilities:

Assess patients and coordinate care effectively.
Plan and execute discharge processes.
Conduct admission reviews using Inter Qual criteria (MCG for ED in NorCal).
Manage a case load of up to 25 patients.
Participate in weekend rotations.

Qualifications:

Familiarity with Inter Qual.
Experience with HMOs, IPAs, and other healthcare organizations.
2 years of acute medical care management experience.
Proficiency in computerized documentation.
Epic experience preferred.

Preferred:

Understanding of adoption and foster care systems.
Awareness of drug and alcohol abuse treatment options.
Experience working with patients experiencing homelessness or mental health crises.
Knowledge of advance directives, POLST, and protective services processes.
